U. S. Citizens (or U.S. Nationals) born inside the United States
If you are a citizen of the United States (or its outlying possessions) and you were born in the United States (or its outlying possessions), you must provide one document from column A or two documents from column B. If you present two documents from column B, at least one of them must be a government-issued photo ID, such as a state-issued drivers license, military ID card, or state identification card.
If you have legally changed your name, you must provide a court-ordered name change document in addition to the required documentation from List A or List B. For example, if you present a U.S. birth certificate with your maiden name and a drivers license with your married name, you must also provide a marriage certificate that shows the link between the names on the birth certificate and the drivers license.
Note: U.S. Mariners who hold or intend to apply for U.S. Coast Guard issued-credentials should note that they may be required to provide the Coast Guard with copies of additional proof of nationality and/or citizenship. List A one document
List B two documents (with one of the two being a government-issued photo ID)
- Unexpired U. S. passport (book or card) demonstrates U. S. citizenship
- Unexpired Enhanced Tribal Card (ETC) demonstrates U. S. citizenship
- Unexpired Free and Secure Trade (FAST) Card designates U. S. citizenship if indicated on the FAST
- Unexpired NEXUS Card designates U. S. citizenship if indicated on the NEXUS
- Unexpired Secure Electronic Network for Travelers Rapid Inspection (SENTRI) Card designates U. S. citizenship if indicated on the SENTRI
- Unexpired United States Enhanced Drivers License (EDL) designates U. S. citizenship
*Valid Proof of Name change (If needed)
- Unexpired drivers license issued by a State or outlying possession of the U. S.
- Unexpired ID card issued by a State or outlying possession of the United States. Must include a State or State agency seal or logo (such as state port authority ID or State University ID).
- Original or certified copy of birth certificate issued by a State, county, municipal authority, or outlying possession of the United States bearing an official seal
- U. S. military ID card or U. S. retired military ID
- U. S. military dependents card
- Expired U. S. passport (as long as the exportation date does not exceed 12 months from the expiration date of the enrollment) demonstrates U. S. citizenship
- Native American tribal document (with photo)
- U. S. Social Security card
- U. S. Military discharge papers (DD-214)
- U.S. Civil marriage certificate
- Unexpired Department of Homeland Security (DHS) / Transportation Security Administration (TSA) Transportation Security Identification Credential (TWIC)
- Unexpired Merchant Mariner Credential (MMC)
Original or certified Copy of a Court Ordered Name Change Document (to include marriage certificates and divorce decrees) if the name on your photo ID and citizenship status document do not exactly match.

You can have the application mailed to your home. They will not mail your card to your home. There is a rather lengthy process involving fingerprints and presenting your drivers license in order to pick up your card. They will call you into a room and you will stand in front of a computer monitor with the fingerprint scanner. The technician will have you fill out some information on the computer and then scan your fingerprints and your drivers license. When all of this matches up then he will give you your card. It takes about six weeks for the card to even get to the facility for you to pick it up. But under no circumstances will be mailed to your home.

remember this, when picking up your twic you can park in the road with your 4 ways on while you go in and get em.

Can i hijack your thread for a second just to ask nd this may be helpful to others:if transfering your cdl from florida to kentucky do i need to retake the tests again including the endorsements is hazmat tranferable as well or do i have to go through same process again

I am probably going to be getting hazmat and a twic card within the next few months, can you explain why getting a hazmat was a hassle? Dont you just fill out the documents and hurry up and wait?

Was not a hassle for me, it is unattainable as I do not have a CDL as of yet, it is required that you possess a CDL, or a permit, as some here have stated, before you can apply for the HME blessing from Homeland Security. So if you do not have a CDL/permit you will not be allowed to apply for HazMat, the biggest "hassle" is that you must apply in person for TWIC and or HAzMAt.
