Find a local insurance broker focused on trucking; for the past 10 years ours has followed the market and has searched out two or three choices to find us the best price. Issues/adds the Certificates as needed and doing all our UIIA filings, etc. AND runs all our MVR's for free...9 units versus 120 units, not a big deal but a good broker beats shopping on this board IF you find a good one familiar with this crazy industry.

I use 925 Partners insurance agency. They are registered with UIIA. they helped me get away from progressive and even saved me some money. Ask for jon or tiffany. they are good

You need an agent that is registered with UIIA, once you get an agent code from UIIA give it to your agent, they'll use it to log into the insurance part of your account and they will fill out the Accord 22 and Accord 23 forms on the account, once insurance is done, your agreements will become active.
Now for Cargo, your agent will shop around for you, I myself have Lloyds of London for Cargo, I had to switch to them as my original cargo underwriter does not provide coverage for intermodal. aside from Cargo you'll need general liability, interchange, and depending on the steamship lines or rail you want to have agreements with you will also need worker's comp. CSX for example requires it, some require a carrier's coverage policy too.
Be ready to take some rape, it will be expensive.
