Yikes! I lost one today too, the plunger stuck down somehow and there was about 2 inches between the top of the injector and the rocker arm. Didn't sound good. Anyone seen that before? Good luck superhauler

Is this just pre acert injectors ? I know a lot of acert powered trucks around my company and can't say I've heard injector problems brought up at all. Are we just getting lucky ? or are you guys dealing with older models ?
TimsItch Thanks this. -
not sure about superhauler, but yes mine is a 1996 3406e

common reasons for failure
IMPROPER FUEL FILTER OR FUEL FILTER MAINTENANCE - High efficiency 2-micron fuel filters must be consistently used. Improper fuel filters will cause excessive wear on internal injector components, shortening the life of the Unit Injectors.
RUNNING OUT OF FUEL - Damage to the Unit Injectors will occur if they operate without a fuel supply. Fuel is needed to lubricate internal injector components. Usually the injectors for cylinders 6, 5 and 4 are damaged (in that order) when the engine is starved for fuel (this is because the fuel manifold become void of fuel over these injectors first).
HIGH FUEL TEMPERATURE OPERATION - High fuel supply temperatures will decrease the lubrication properties of the fuel and cause damage to the injectors. If the fuel return line (to the tank) is blocked or restricted, very high fuel temperatures will be reached in the fuel manifold. Also if the fuel return line remains blocked to the tank, a build-up of air bubbles will occur in the fuel manifold. This trapped air will usually result in damaging injectors for cylinders 6, 5 and 4 (in that order) first. (depending on flow direction through the head) This is because trapped air in the fuel manifold occurs over these injectors first.
Note: A 174-00 High Fuel Temperature Warning code is triggered when fuel temperature is above 90° C (194° F) with the engine speed above 1000 rpm.
COOLANT OR WATER IN FUEL - Coolant or water in the fuel will cause damage to the injectors. This condition is usually due to a failed Unit Injector sleve.
INCORRECT INJECTOR HEIGHT - An incorrect Injector Height setting (lower than spec) may cause the injector rocker to bottom out the injector plunger into the unit injector. This will decease the life of the unit injector.
INCORRECTLY INSTALLED UNIT INJECTOR - A loose or improperly seated Unit Injector can led to injector failure. This may also cause O-ring seals to fail and combustion gases to leak into the fuel manifold (Air in Fuel). Leaking combustion gases may cause damage to injectors "downstream" (also performance complaints such as stumble or rough running under load).Last edited: Apr 20, 2013
