There have been a few Swifties written up for not carrying the long form.

Swift passed CSA2010 with lots of room to spare. Our two biggest issues were (41% of writeups) driver did not have long form medical in possession and (40%) driver did not secure cargo...the spare tire was in the rack, but the chain provided with the rack was not latched or hooked in any way.

I never even received one of those little cards you refer to and had never even heard of it until just now, when you wrote about it. One would assume (yeah, don't say it) the CDL itself is proof you passed the written and practical tests.

In Calif. the DMV keeps the Long Form and we get a Green Medical Card. I know some states do require it,but I haven't had any problems.

I have heard that you need to carry your long form with you before, but I always put it down to rumor.
Never had a company say that I needed it with me, and I have been through inspections in Cali, PA, NY, ME in the past couple of years, and none asked for it.
I believe there is still a rule saying that you have to have a card showing proof of road test and written tests.
I am not sure though. I got them when CDL was just coming out, but that was almost 21 years ago lol. So I do not know if they are still used at all.

I have to stop reading this thread now. I got pulled over the other night by a young trooper who was probably behind on his activity due to bad weather. I felt bad that there was nothing he could get me on easily I was tempted to ask if he had a creeper so we could try to find something. And all I could think was "Deiselbear is that you?"

Last night I passed a trooper and 2 mins later blew a tire. Second time in recent memory I passed a trooper. Last time I passed a slow moving DOT trooper that was scanning the trucks and got an inspection for a marker light out. I'm going to do it once more for the "third time is a charm" and see what happens. lol :Trailer: + :Police car: = :Automobile loan:

Its called your photo ID commercial drivers license.
They come in color now days. :biggrin_25523:

In all the many inspections I got over the yrs I was asked for "YOUR MEDICAL CARD".

NEVER ONCE "your long form physical".

That is private medical info. All the DOT officer needs is signed and dated proof that you passed your physical by a certified DR. The rest of it is none of their business.
