    JJ Keller makes cards. I have seen them in truck stops. Look up "occupational medicine" and you should get lots of hits for clinics.

    Not sure if you've gone already boog, but went for mine today and they supply everything for you. Tell them it's a first time DOT physical and they have to provide the long form (2 pages for me) and the card. Hope that helps
