    I had a surgery 2 years ago to repair a broken colar bone when I was in the Navy I just seperated and I have been doing fine will I need any kind of documentation that I was release fit for full duty or does werner has their own form I may need my Surgen to fill out

    Most companies would require you to get a medical release from the dr. who treated you. Tell them up front about your injury, and they will instruct you as to what you will need. You will be required to have the dr. say full duty 100% no restrictions. Hope it works out for you!
