I worked in the CATV industry for over 6 years. Trust me nobody cares except your safety director. Trucks rip down lines everyday. There's to many variables that can cause a line to hang low. Was it hot? windy? did a bigger object catch it first? They get ripped down you just replace it and move on. Now if it's plant (hard jacketed) it's still not your fault if its under 14' but that will bring more attention for sure.
Sucks to hear your getting a ding for something that happens hundreds of times a day.

Preventable/nonpreventable has nothing to do with who's fault it is.
It's about whether or not reasonable steps could have been taken to avoid a situation.
If a wire is visible and a person drives under it not knowing if it is going to clear their trailer, that is a preventable situation. -
