You need to pull out the MVR on yourself. You need to do the pre-employment testing to be screened for drugs/alcohol. You need to show you are enrolled in a consortium. You need for example a list of employees that are signed for a random testing in your company, a contract with the third party, the form you need to keep in your truck if they pick you for the random testing called Custody and Control Form. If you did the pre-employment testing, you need to submit the results.
You need a trailer/truck annual inspection.
You need to have a list of drivers, a list of trucks and trailers with the VINs. They copy of the ELD device model, etc.
They asked for a 30-day log with some proof of the trips you took like BOLs or fuel receipts to go along with your submission.
INSURANCE!
You need to include the MCS-90 form from your insurance carrier.
Reportable crash report for your driver/s
Medical Certificate
You need to provide license plate numbers. If you did not renew IRP, submit a copy from your state showing an extension due to COVID-19.
COPY OF THE CDL driver's license. Whatever you didn't renew due to COVID-19, make sure your state exempts you, for the time being, attach it.

Due to Covid-19, they might not require the pre-employment drug test. But it depends when you opened up. If you were open for a while before the COVID-19, they might be hard on you. If you opened up during the pandemic, I don't believe they require the pre-employment testing. But if you can do it, I wouldn't wait.
If you show a complete list, they might be a bit more lenient by checking your logbook, etc. or whatever you might be missing.
Try to submit paperwork as complete as you can. I would start now and wouldn't wait.
I was advised to submit only what they are asking for, no more. So if they don't ask you for a copy of the employment application for your driver/yourself that you need to keep in your file with the history of employment, etc., don't send it to them.Last edited: Jul 14, 2020

I have liability insurance but dont have a truck or my cdl yet. I applied for my authority already. Am I going to fail? I asked and everyine said it was ok if I didnt have a truck all I needed was my liability insurance. Ugh
