Here is the information on how to submit your medical card in Ohio. You can mail, fax, email or do it at the BMV. I believe they charge a fee at the BMV.
All CDL holders who self-certify Non-Excepted Interstate and submit a MECert are required to keep a valid medical card on file or modify the self-certification on file if their type of driving has changed.
The Ohio BMV will send a notice approximately 60 days prior to the medical card expiration to inform CDL holders that their medical card is nearing expiration. If a new medical card is not provided, the Ohio BMV will mail a notice to the CDL holder indicating the MECert is expired.
Once the medical card expires, the driver is no longer eligible to operate a commercial motor vehicle. In order to become eligible to drive a commercial motor vehicle, the operator must submit a valid medical card or change their self-certification. If a valid medical card or change of self-certification is not submitted within 60 days, the CDL privileges will be cancelled. If a valid medical card, change of self-certification or downgrade of the CDL is not completed within 180 days, the operator's license will be cancelled. Once a license is cancelled, the knowledge and skills tests must be taken in order to obtain a new operator license. Additional knowledge and skills tests will be required in order to obtain a CDL.
