It happened in my business years back, one of my drivers left the PTO on with the hoist lever in the down position but the valve must not have been completely closed and oil bypassed and raised the bed.
The driver just dumped off a load of stone and was traveling north on route 295 at a speed of 65 mph not knowing the body had raised completely according to 2 witnesses given statements to the NJSP.
The truck hit a overpass and as the truck raised from the force the body snapped off and landed on a car behind the truck, the chassis of the truck did 3 backward flips landed upside down skidded 350 feet and uprighted on its wheels.
The driver of the car that the body landed on was not injured, My driver was also not injured but stayed in the hospital 2 days for observation due to not remembering what happened.
The NJSP never found the piston from the dump body.
Also the bridge was not damaged.

Couple of weeks ago a guy at our shared yard ran his raised dump into their fuel island canopy. ####ed it up, royally.

Sometimes you just shake your head.
