PA Self Certification form

Yes, I received an acknowledgement letter from them about a week after I submitted my information.
The contents are as follows:

It is "signed" by some bureaucrat and has a bunch of contact information. I filed it with my long form original.

 

I mailed mine in for NC and have yet to receive back anything. This was about 3 weeks ago. I did keep a copy and date it the date I mailed it in. I guess I should call them and see if they are going to do the same for the receipt of this letter.

 

I was given one of those in NJ, I have no idea what it is and what to do with it. I asked the lady at the DMV and she said keep a hold of it, so I still have it, do I have to mail it in when I have passed my tractor driving test?
