Like to stay away from opinions on the way they look and fit/finish.
If you were to pick between say a 2011/2012 386 and prostar both with a isx both around the same milage bothe strait 10's which would you do?

Peterbilt because I truly think that you can get better fuel mileage out of them and they weigh less than the Prostar.

Depends if you want resale value in 3-4 years, or if you just plan to run it till it's dead. Personally, I prefer Internationals, but I know the Pete will hold value better.

I honestly like a prostar/isx combo. I've had good luck with them, 6.5+mpg and I am familiar with them. I detest kw 660's looks but like the 386 and I know the 660 is probably the most aerodynamic truck on the road and figured the 386 got some of those traits from it's cousin.

Prostar is a 2012 with 325k miles on it, 450hp isx, eaton fuller 10 speed. $39,000
Pete is an 11 with 370k miles on it, 450hp isx eaton 10 speed $44,000
Have seen a clean rig dig on both, both have regular updates on ecu history. Prostar had a new turbo at 275k -
Between the two, the Pete was a better ride. Far as fuel mileage they both did around 6.9 mpg pulling an average of 48k. A lot of cruising and minimal hills.

If you like to work on the truck yourself, that 386 having the longer hood makes working on that Cummins engine a HECK of a lot easier. The back of the engine is accessible. Prostar has it crammed up under the cab pretty far.

Apu? Bunk heater? Similar bunks? If all else is equal, I'd go with the Pete.
