    Check some local urgent care centers. Some around here charge $25 for school or DOT physical.

    The DOT physical forms are online, you can print your own.

    With forms in hand, some local clinics are almost free.............

    yes, but however, "some" companies DO NOT accept the paper card. they want the cardboard DOT card. i have had a few of my students get rejected because of the paper card, and i'm also talking about the DMV road test troopers, that WILL reject the paper as well.

    best to just do a web search for a trucking association and buy some blank DOT forms , and have them shipped to you.........or, go to the clinic.

    but then frankly, i see no real good reason to buy a few in advance. the questions on the form may change at any time, now with sleep apnea testing.

    Exactly right. Most health insurance allows one annual physical.

    Just let your doc know ahead of time so they can print out the forms. Not all family doc's know what is required for dot.

    That is why it is good to know the regulations pertaining to DOT physicals.

    That little green book can save a driver some cash !!!!!

    the DOT card must be on card board (card stock), NOT paper..........


    • Purchase a separate "medical card" from an industry supplier of motor carrier forms (e.g., State trucking association) before the physical exam and ask the medical examiner to complete and sign it, in addition to the Medical Examination Report form; or
    • Print the Certificate on cardstock paper. It is permissible to reduce the size of the Certificate.

    i HAVE HAD some of my students get told, the paper DOT card IS NOT ACCEPTABLE.....

    a company has the right to refuse a printed "card" on paper...........

    AND WE HAVE HAD Mass troopers REFUSE the "paper card" as well...........

    and something else............NO card, paper or card stock, can be laminated.......UNLESS it was laminated by the doctor's office..........
