The first thing they do is send you through their own doctors. They do not trust anything that you might bring in.
And for good reason, as ZVar pointed out.
It is simply too easy to falsify that document.
Any decent printer/scanner with a good graphics program makes the job very easy. -

In all honesty I don't get the big problem with physicals. Just go get it done. If you have issues with BP and diabetes and can't get at least a one year card, having to get a new physical is the least of your worries. I know white coat etc etc however most clinics will let you relax for a few minutes. Just relax. Just something as simple as not drinking soda's for 30 days can account for as much as a 25 point drop in the big number on BP.

You know what I will be getting my physical soon, I think what I'll do is wait about 20 days to take it to the BMV. Push the 30-day envelope but not all the way. And when I go there I'll see if it was already put in. Normally I take it within a day or two to the BMV. This time I'll give the system time for it to be input before I get there. -
In my experianced, it depends on if the Dr office is set up to enter it in. The Dr I went to when I went with national strategic was fully automated with newer medical forms and no actual cards, the Dr I went to for transport national didn't have a clue about the new forms, and doesn't enter anything into the system. Ohio allows you to fax your card to the state Bmv office, but its best to physically go to an office and enter it in yourself

It is still better to bring it in person, just in case they claim it was 'lost' or they never got it.
That is why I always take my home time when I need to take my DOT physical. Because then I just head straight to the DMV to submit it, and I know it is done.
I don't think any doctor sends that info in to the DMV for you.
And you still need to fill out and submit the self-certification form, and the doctor does not have those.
