If Swift will accept a current medical card, even with the long forms, things have changed.
The first thing they do is send you through their own doctors. They do not trust anything that you might bring in.

And for good reason, as ZVar pointed out.
It is simply too easy to falsify that document.
Any decent printer/scanner with a good graphics program makes the job very easy.

Was at swift in 2016 they took my current med card then.

It is, in fact, much more advanced than that Star Trek device.
