I'm on Prime's Olney dedicated account. There are two groups of drivers: Those who are hired directly to the account and work local/regional, and those of us on "Division 111", which is what I'm on. They are basically the same and share the same work. Dedicated are slip-seat, go home every night, and can even pick scheduled days off. But they have to live within 50 miles of the Distribution Center. Division 111 drivers are technically OTR, so we have assigned trucks, but we also follow the same hometime policy as other Prime drivers.
Delivery radius is about 250-300 miles. Farthest north I've delivered to was Bloomington, IL, though once I pulled a backhaul out of the south Chicago area. We mostly run IL/KY/TN/IN with some stops in eastern MO. Pay is per mile, but you get about $15 for every stop on your route. I average $850-$900 each week after taxes. You can make a little more on the OTR side, but I prefer this since it's consistent work and I know what to expect. I live in St. Louis, so whenever I get sent that way, I just take my 10 at the house.
Hope that answers some of your questions. -

I use an external insurance plan, so I would not be able to tell you.
