Rule about where to position a single steel coil on a flatbed

But if you "place coil here" how in the world do you secure it to the frame rail?

It's not 1, else the trailer wouldn't have the damage it does, and the coil wouldn't be next to it.
Since the back pins look like they're still in, but the front pins on that sliding tandem axle aren't in, it's possible they weren't engaged properly, thereby allowing the trailer flex it shouldn't have had. Other than a little rust, (perhaps because of the picture quality and blur) the trailer looked like it was otherwise solid.

I'd have to guess a manuever (perhaps trying to make a tight turn or wide turn) with a possible disengaged sliding tandem axle pin, it looks like the trailer started flexing, loosening the securement enough to let the coil shift enough to make it heavy on the driver side and buckling the trailer and further pulling the front pin axle away from the trailer. It could be bad loading, since it looks a little further back, but that could be photo angle too, can't be sure. It does look like it's within the coil pack area.

I am going to guess a combination of #2&3. Driver made an evasive manuver and the trailer couldn't handle it.

 

I say #1 sir. looks like the coil moved to the left side of the trailer, once it wasn't centered the trailer couldn't handle it anymore and mayhem insued ! I can only see one chain and strap in this pic. Assuming he was hauling it shotgun to begin with, I would think he would have had three or four chains on both side. Hard to imagine them all being hidden in this pic, but maybe he grabbed them up before the pic was taken.

 

I count five chains and two straps, all of which seem to be be coming from the passenger side of the trailer.

 

I always places a single coil 4 ft in front of the lead axle on the spread.

 

Coil doesn't look super heavy. Location on deck looked ok. Why isn't tandem attached in front? Must have been driver slide misfunkup there with the pins. And possibly chains came loose since coil is still not in center of trailer.
