DOT medical form ask about prior surgeries and neck or back problems. Be truthful with your answers. I had a herniated disk in my neck from my factory job. When I started driving I did not hide it. I have no complications from the surgery, and now work for a great company with excellent equipment. If the company you are looking at has good values, your honesty should go far with them.

I have scoliosis and 3 pinched nerves in my back not a one person affiliated with a trucking companies management OR doctor has ever been told besides the chiropractor that found the pinched nerves..(one of those 25 dollar first visit deals)

Also, it would not be a bad idea to drive for a company that has all automatic transmissions, to avoid stress and strain due to both shifting and pressing the clutch pedal on a frequent basis.

If the company is asking about spinal injuries, it must mean there's some heavy lifting involved. It's your call, but I'd go with a company that has "driver no-touch" freight.
With that question on the application, it's a good bet the doctor doing physicals for that company will raise your shirt and look for surgery scars.
A few companies will accept your cdl school DOT physical card and will only do a drug test and maybe a blood pressure test, but no physical. They will want a copy of the physical long form though in most cases; it's required for their company files.Last edited: Mar 18, 2017

No point in lying about an injury if the application asks for it.
True, medical history is private, however filling out an application is voluntary.
Leave it unanswered if you really want to duke it out with HR and insist on keeping that information private, but you probably won't get hired for not playing ball.
But if really do want to work with this company and you don't want to get canned unexpectedly at a future date, sooner or later, then answer honestly and like others have said, have a doctor's clearance for work handy. -
