I've already got my DOT health card and start school next month. When I get done, find a carrier, do they do a physical as well? What happens if I have sleep apnea? Am I fired on the spot?

Most major carriers, and especially those who train, will require a medical be performed by their preferred doctor group. The likelihood of being flagged for a "sleep study" is hit and miss, but if you have reason to suspect you are a candidate, their doctor will likely pick up on this, too. What they may or may not do at that point is another question.

If you're concerned about it, hire on with a company that will accept your current DOT card. They will probably want the long form or a copy of it. Call and ask if they want a long form copy.
Here's some companies that don't require another physical and will accept your cdl school physical:
TransAm
Paschall Truck Lines
Freymiller
Magnum Ltd.
J&R Schugel
Put Paschall at the bottom of the list. The other 4 are ok.

If they suspect you have sleep apnea, you will have to get a sleep test. Some bigger companies do it themselves. First part off test is to see if you really have sleep apnea. If you do the second part is correct the sleep apnea. Something nobody ever talk about is everyone has sleep apnea. The way they count sleep apnea is Events Per Hour. For an event to count it has to be 10 seconds long. So everyone has 4 or less Events Per Hour is considered normal. You start going 10 and higher it becomes more of a impact on your sleeping. 12 event x10 seconds is 120 seconds every hour of not breathing. That's 2 minutes. You 45 Events X10 is 450 seconds or 7.5 minutes per hour of not breathing. That's why if you might have it, you want to know. Try holding you breath 7.5 minutes every hour.

You live in Tennessee; my home state?
Probably get a used CPAP online or Ebay pretty cheap. Maybe research Google on this. Try to get one that has the capability to download data. I think the DOT doctors want 90 days data to prove you use the thing.
