It is rare, but it does happen, not all tickets make it to a driver's DMV record. Had a retired trooper, DOT officer tell me that humans are still involved in sending, receiving and posting violations on drivers record. And when humans are involved, slip-ups, mistakes and omissions will happen.

I had a DOT trooper write me up one night for a headlight being out. Never did that ever show up on my record in any shape or form. Company never got hammered with CSA points.
