The phy that you take will cover all that but here is a list for bp that could affect you bp 140/90 159/99 the second stage 160/100 179/109 stage 3 180/110 or greater and then you pee in cup they check for high levels of protein and sugar i dont think diabetes will stop you unless its not controled by pills or insulin the bp is the same as long as it is controled just take any meds you have with you all this comes from the list i got from my rec

Swift does have a list. I know this as I use to have one. I no longer have a need too. I do know Swift prohibits certain medications. In Milington, they give you less than 5hours to get everything resolved if you do not have the items you need. My suggestion is visit your doc's before with this list. Tom S., the processor in Phoenix should be able to e-mail you or fax one to you.

7702003 I know you are on medication other than insulin, but IF that test that I can not remember the name of shows a higher than accepted level of sugars in your blood over the last 6 months, you won't get a card until it is controlled and then probably for only 6 months. I was diagnosed seven or eight years ago with type II, but it has always been under the percentage allowed so I have a card good for a year.

its called a hemoglobin A1C, its a average of daily blood glucose levels.
