Nope they were never off the rim. Yeah I am not too disappointed in the good years but would like to prevent. This odd wear from happening again

You may be running too low.

My Goodyear steers did the exact same wear pattern and the Toe was way off, got an alignment put on new Michelin steers all is good now. 125k and wearing perfect.

You have too much tow-in. Shocks worn maybe as well. It probably is very skittish going down the road.

Replace ALL your shocks, most of the time this is caused by the drive's shocks being bad.

Front wheel bearings. Also, if that tire ever came in contact with a curb, and we've all done it, it can break a belt and will never balance. I'd check the wheel bearings, but right now, that's a trailer tire, as once they start to wear like that, it continues.
