There's another quarterly performance bonus Swift has, based on your average miles per week, on time performance, and safety record (based on preventable accidents). IIRC everyone starts at Bronze, then Gold, then Platinum. To get Platinum you need to average at least 2100 miles per week, be 99.5% on time, and have no more than 1 preventable. The Platinum bonus is $0.04 per mile for all miles during the quarter.
I had several trainees achieve Platinum status within three months of going solo. The bonus can add a few thousand to your annual income. My last year with Swift as a trainer it was about $7K. Stay Platinum for three years IIRC and you get Diamond status and the bonus kicks up to the $0.06 to $0.08 range.
Platinum also allows you to select from up to three different load offers. Planners also pay attention, if they have a critical load they will give it to a Platinum driver instead of Daylight Danny that don't run at night. -

I love driving nights. Less traffic and more miles. In my current gig a high power hand held spotlight is essential. Running 20 miles of dirt roads to find an oil rig can be a challenge at night, especially when you are navigating with "turn by turn" directions that say, "Turn right on FM 1270 then turn left before the third cattle guard after the abandoned farm on the right". Happiness is seeing that lit up rig in the distance. Unhappiness is realizing you must have made a wrong turn and spending at hour getting a split axle flatbed turned around between the trees and swamp.

You also have to break down mileage pay. PC miles? Hub miles? Just made up miles?

The things you listed are called accessorial rates and every company should have a list of them with the payouts that the driver should always get.
