Cat has really let there qc go to heck.Head bolts to spec at 350lbs,it makes the head gasket wear early on the rear cold side corner.When we changed the cam we went ahead and cut the block put a new headgasket in torqued it to 400lbs this will thwart a bad headgasket.Another reason engines dont last is egr,cat has there own version of egr in a water cooler,bottom right side,just left of waterpump.There was a ton of carbon build up in stock manifold,kinda like a yellow version of a detroit minus the vpods and veined turbo.Materials in the engine are cheaper as well,the steel isnt nowhere near as strong so it wears easier.When you rebuild one swap heads with an older engine if possible it will last a lot longer,on another note if you torque those head bolts to 400lbs you will see them swell and not take the abuse.I had four that couldnt cut the mustard innitially.When you get a good engine keep it forever,when you know what your dealing with life is easier.cheers
