UCR application for new carrier

Hey guys, I'm stuck in a endless loop with my UCR applicaton. Here are all issues:

- since I am a Canadian carrier, I need to choose some US country as Principal Business State. I have 2 issues with that: different instructions online and from lady in NY office- should I choose US country that borders with Canada or one of the listed in help on online application (for example, Michigan is not listed there). Second issue- what to list as a zip code (I don't actually have one since I'm in Canada)

- online form (http://www.ucr.in.gov/ ) won't let me punch in date on last page (yes, I am using I Explorer with low security as advised),

- I don't know where to send manual form to (did try calling Ilinois FMCSA, keep reaching voicemails), is there any fax / email
