Fuel mgmt. is most often done by setting engine parameters to save fuel and limiting truck speed settings.

Bracketed pay rates set to fuel mileage has been tried, but it's not fair when drivers have different routes, some of which use more fuel.

Fuel bonuses don't work because they are too small. Drivers prefer to get home sooner and idle to be comfortable, rather than receive a $50 bonus every month.

APUs or at least a bunk heater will cut down on the idling and still keep the driver comfy.

A fuel mileage competition with a prize works better than just a fuel bonus. Drivers like to compete and it keeps fuel economy in there minds the more it is discussed.

Also keep drivers in the loop concerning company profitability. Setup a profit sharing program where the most profitable drivers get the biggest annual bonus. Weekly meetings discussing safety, service, profitability, and who's ahead in their fuel economy keeps drivers thinking on track while giving them a sense of being part of a team. Listen to your drivers input and encourage new ideas.
