In my case a hydraulic 60gpm pump mounted to the transmission, that feeds an air compressor or a blackmer liquid pump. Your application depends on what you need the pto to do.

My 2016 already had the PTO prep kit installed from the factory. It included slightly different location of the air tanks so there was a clear line of sight from the PTO mounting point to the rear.
My local dealer spec'd all O/O trucks this way as at the time it was the only way to defeat the anti idle regulations.
It allowed me to just flip the PTO switch on the dash and the engine would rev up to 1,000 rpm even though I didn't have an actual PTO.Accidental Trucker and KDHCryo Thank this. -

We've got a 2020 Cascadia needing a wet kit that was not PTO prepped by the manufacturer. The PTO has been installed but we have major clearance issues with the flange at the output of the aftertreatment box. Do you have any pictures of your setup?

Yeah, that sounds like a PTO to a hard mount driveshaft issue. Meaning you have a driveshaft hard mounted from the fixed transmission output, through a frame mounted carrier bearing to an output pump. Not ideal.
You need a setup like mine where the pump is hard mounted to the PTO output, then hydraulic hoses to the end hydraulic output, if that makes sense.
It's a lot easier to route hydraulic power in a flexible hose than through a driveshaft. It costs more on the front end, but maintenance wise on the back end it saves a ton of money.
