In Washington State you go to the local DMV and request it. When I did mine it cost $5,00 and they printed it out and handed it to me.
Make sure you list everything that's on the MVR, later Swift will have you sign paperwork that will allow them to go directly to the state and get a copy of the MVR and they will compare what you have put down with what they get. If it doesn't match they will drop you without explanation.

Swift orders the MVR themselves, I forgot (some companies make you bring it). Brain dump on my part, sorry. I've been applying to a bunch of locals over the past few months who have all wanted me to bring them a copy.
Once again, I apologize for my inaccuracy. On the other hand, you should keep check your MVR and licence status yourself on occasion. Little story behind that: One day on a wild hair, I checked my licence status online (Free in WA state) to find my licence was suspended!!! Turns out when I paid a ticket online, it posted from the bank 1 day late causing an extra fee. Since the extra fee didn't get paid, the ticket went into default... licecense suspended. All straightened out, but apparently I drove suspended for a month. Luckily, I got away with it. That would have been expensive had I been caught.
