Let me get some opinions on documents that need to be carried:

Paper documents relating to the load, it's contents, it's origin and destination location.
Paper documents relating to the driver's license, insurance, and DOT physical (can these be electronic and kept on our laptops or smart phones as PDF?).
Emergency contact information in the event of a collision that leaves the operator unable to communicate.

DOT card must be physical, insurance i would assume be physical,DL has to be phiscal.
emercyny contact keep with your permit book(inside door pocket, above DS seat) so its easy to find.

For emergency contact info, it's a good idea to put it in your cell phone contact list. Pre-cede your emergency number with the letters ICE (In Case of Emergency). Emergency responders will check your phone for this when they are trying to find who to call.

Truck registration, IFTA license, weight distance permit (NM), and proof of insurance should be physical and easily carried for POE (state Ports of Entry) and DOT inspection/review. Photo copies seem to work fine.

I have mine in a clear plastic menu holder.

I have my Manifesto, I'm almost half finished with it.
It's proving hard to fine the correct ribbon for my Smith-Corona typewriter.

I have attached an emergency information sheet that was adapted for use from the motorcycle club I am part of as a template. What other information should I include?

Based on suggestions, I will add spaces for the additions and repost for anyone wanting to use it.

I carry the title to my truck and trailer. Never know when an deal comes up that you cant pass on. Then again I could get a bad attitude sell it for a little of nothing jump on a plane and go home. After ELD is forced on us, the second scenario is a real possibility.

Wow! Lol @ mtoo for the carrying truck title. I had a laugh at that that, lol.

I'm a company driver and this is what our truck folder consists of :

    - Truck Registration
    - IFTA License
    - Annual Truck Inspection Sheet
    - Certificate of Insurance
    - NM Weight Distance Permit
    - New York Credential Certificate
    - Photocopies of both my D.O.T PHYSICAL + CDL (of course i have physical copies of these).
    - Packet of all reefer registrations that my company has provided to us just incase a driver misplaced or took the reefer registration that's supposed to be in the hub infront of the trailer.

EDIT: Oh.. besides the B.O.L when asked...

Thats about it. Never had any troubles with the troopers.
