My 2017 t680 with a isx15 doesn't have a coolant filter and I am just over 500k miles on it now. I had the truck in the shop to do a few things and had them do a coolant change or radiator flush. When I got the truck back it had this thing on it off that cap I didn't notice until I got it back. I'm sure it's simple but I've never seen one and don't know what the point of it is?

Looks to me like they just plugged your over flow hose. Maybe to pressurize cooling system to purge it or just pressure check for leaks.

There should be a hose attached there that goes somewhere I would imagine.

Should that be plugged up like that or should it be open since it's an overflow?

I would say it should be left open and routed down to the bottom of the rad.

That bolt will never seal it off 100% anyways.

Maybe it’s their test cap. Your caps back at the shop. Someone’s tool is on your Truck. My guess.

More then likely they used a vac n fill to fill the coolant system. That vent on the side is the cap for allowing excess pressure or vacuum to dealt with during normal warm up expansion and cool down contraction.
The cap on top is just a cap. The vac n fill (air lift) etc won't work unless that vent is capped.
That is a shop tool per se. I'd remove and discard it. There may have been a hose attached to route coolant overflow to the bottom...
    Ask the shop...
