My DOT physical is only good until November of this year. Do I need to get another physical in my CDL state?
In my otr job I don't even go through my cdl state, so it would be better to get the physical done near the company terminal if permissible.
Where to get DOT physical

Your company should send you to their place. You don't have to be in your state. Just ask them.

You can go to any place that is registered to do DOT physicals. You will need to go to your home state motor vehicle department to give them a copy of your medical certificate. They will downgrade your license if you don't before current one expires. Make sure they give you a receipt that they received it.

Most any "occupational medical clinic" do them at a mostly reasonable cost. Many carriers will reimburse this, some not if you use another doctor. And be sure you go through the necessary steps to insure your state's DL division gets notified of the new medical certificate or else your DL might be suspended. Check your state's web site for instructions how to "self-certify" a new medical.

Check your state's DMV website to see if you can submit your med card online. Florida accepted a picture of my med card that I renewed in Virginia.

Google maps. DOT exam. Call and ask price. Anything from $80 to $220. Last I paid was 80 before that $130.

Cvs does it. It's about $150 for everything.
Make sure you get all the required paperwork from them.
They didn't sign one form of mine so i had to get it done again, luckily it ended up being free through the company. -
I’m due for one of these too. My out-of-state company does it for free at their doctor of choice. But I’ll need to be sure my home state has a copy and is updated.

What was that big chain drug store... I remember them offering DOT physical.
I can't think of the name ATM but they are all over.
