You did not answer the question.
If you need four men to run a garbage truck though they ought to be able to help each other get in and out.
Seriously though, getting in and out of the old cabovers that I drove was not an issue. I much prefer a conventional to drive though as I like the feel of the road better in them, and I especially like narrow cabs. And I never did like the mushy shift linkage or the really short shifters.

The floor, flat or not, is over the engine.
The doghouse is over the radiator and charge air cooler and maybe some other components such coolant expansion tank.
All of this is higher than the engine itself.
Many euro trucks use same layout:
DAF MX, clear to see the ducting from turbo to charge air cooler. Higher than engine itself. Not the best picture, but all I have for now.
