If it was (In fantasy) a cabover, the sleeper would be so big, there could space for a shower and a toilet. (But no excess to the engine) -
Yearly safety and emission test are in general done by certified truck dealer or independent workshops with a random check by the authorities after the workshop reports that truck is OK.
Some of the shops who tune the engines also do the yearly emission check.....

is there anything else on usa market similar to mack lr?
i have spend today few hours today with mb cab in front of engine truck. it is made to create best possible work environment for garbage trucks.
cab is moved forward and placed low, to allow better access, it use bus style doors, so operator just walk in, there are 4 seats inside and cab is walk trough type while 2 seats are placed above engine. since driver sits low blind spots are eliminated fully. windows are everywhere so you can see everyone and they can see you. 6 mirrors, 5 cameras, sensors for persons/vehicles.automatic braking etc.
it is bit strange to drive for me, it is 6x2 truck and steered tag axle. overhang is equally long as wheelbase. you are sitting in front of axle and when you turn steeringwheel you are bit confused when to steer, and how much to steer, feeling when and how much to turn you have based on COE will result with very big movement of its tail (rearmost is narrower than rest of body) since you are sitting low you cant judge where is your rear low corner of truck like you will see it from high placed cab -
