Memorandum of Record stating the type of equipment driven and for how long.

DA 348 - Equipment Operators Qualification Record

DD 214 - Certificate of Release or Discharge from Active Duty

Requirements:

We can only accept military personnel with M915 / M916 or tractor trailer experience

Exceptions:

Once your civilian CDL is issued, QC will accept your military driving experience in lieu of meeting the minimum requirements listed on the application.
