From your photo it doesnt look that bad but your company will probably put it on your dac as a preventable.Other employers may ask you about it but I doubt that will cause them reason not to hire you.I bent a fareing once while turning around in a tight lot at a receiver only I never told my company and nothing was ever said.But the trk I had wasn't new either.
I bent my cab extension flare, will it go on my DAC??

what i'd do is at least have a sit down open/honest talk with the safety dept. just do not mention you are leaving.
then ANY application you fill out, BE HONEST, especially when you get a copy of your DAC and it IS there.
on DAC reports there will be a section for incidents/accidents.
bending the wing IS avoidable, but it happens. bending a wing weekly or monthly may get someone canned.
so if you have nothing (other than the tire issue), then don't sweat this too much.
the CSA (PSP issue on you) to me is not a big deal as it was only a tire. thing here though, you only have someone's word that told you to go get it fixed. UNLESS it is on your Qualcomm, and you can print that out?
again, in any application that might ask you about incidents/accidents, or PSP points, BE HONEST, but without giving too much information.
