OK. So your wondering if ever disclose my broken back, broken ankles, fractured hip, 3rd degree burns, broken nose ( 3 times ), asthma.
The proper response to every question is no. In fact I just got another two year card a few weeks ago.
There was a dude in there with his entire face tattooed with God only knows how many small tattoo's. Now how can the doctor look at that individuals form and not ask how come he didn't check the box asking if there are any mental or psychological issues?

When I went to get my last physical, I answered the question that I was on no meds, and I wasn't. When I finally got in to see the Dr., he questioned me about it and said my file said I was on "so in so". I told him he was crazy and that I would be the one to know if I were taking meds. I don't even take things like Tylenol over once or twice a year. Then he mentioned it was for acid reflux.
I then recalled that I went to the same office over a year ago with stomach problems. They prescribed me something, I took them until the bottle ran out, and I quit. Never had a problem after that so I guess I was cured. For some reason my records had me still on it and he was trying to catch me in lying on the form. I had honestly forgotten about even going to the Dr. until he brought it up. He still didn't like my answer but finally dropped it.x1Heavy and Dave_in_AZ Thank this. -
I scan through the other responses pretty fast, but if the question wasn't asked, I'll ask it now.
