How about tryiny to resolve the issues that are causing your borderline failing BP?
Lose a few pounds, cut out the high sugar drinks and foods, and a few days before your next physical, stop all caffine intake.
And if you smoke, either stop or at least cut down.
After you see that you don't have to stress over the physical, you lose the "white coat" dilemma.

2 days before. Double your BP meds. Drink lots of water. You will be fine. Plus if your being treated for only HB and DOT examiner should know it can 6 months or longer to find the right combo.

    Be Safe Out There

Cutting down on the smokes won't do it. It has to be total stoppage a few hours before the physical. At least 6.

    To specifically answer your question....

You can have One only. You should have the condition, which caused the restriction, under control by the end of the 3-month window.

With the typical 3-month medical card, you needed to do something to bring some physical condition under control.

If you did that, and you go back to see your last DOT examiner, he can issue you a new medical card for typically one year.

If you didn’t fix the issue, he can choose not to extend the card. So get the condition taken care of and don’t have to worry about short term medical cards.

If anyone gets a 3 month medical DOT card, then they have been WARNED that you have to do whatever you need to do to FIX THE PROBLEM such as high BP etc within 90 days. IF you do NOT do anything, then chances are your problem is still present when you go back and your DOT doc says forget it. No more DOT medical cards.

Every time I check my BP on one of those things at the supermarket that you stick your arm into I always get a high reading. Yet whenever I’m checked by a medical professional I’m always good to go. I think their trying to sell unnecessary OTC BP meds.

I use one of those battery machines and they are pretty accurate. Remember that every BP reading is going to be different.

I take a BP twice a day once in morning and when I do not feel well due to excessive BP. It not only takes BP but counts the pulse and checks the quality of the heartbeat. I have a pulse oximeter that is very close to the same count and assures me that my blood is carrying 95% or better oxygen at room air like it's supposed to do. If I am short of breath and below 90 Im going to the ER before I start to "Bradying down" which is pretty irrevocable.

I am certified on the manual blood pressure and can take them all day on anyone. There is a particular way to use one as a deaf person. The needle or mercury column knows all and will tell you where the two necessary numbers stand. Bring needle up to 210, release air gently at a STEADY RATE. When it starts bouncing that is your top number. Wait for it to continue bouncing and dropping at that steady rate. When it quits bouncing and resumes a steady move near the bottom of dial that is your lower number.

Anyone above 100/60 is considered stable. Below that needs ER, and anyone over 140/90 is hypertension. Anything exceeding say 160 over 110 towards 200 plus over 130 plus is a genuine medical emergency. (Imminent stroke, seizure etc.)

Pulse needs to be resting at 60, no more than 100 under stress. ANYTHING beyond that is excessive. If you came up with say 70 over 40 and a pulse of 130 and rising, that person is close to death in a variety of interesting ways, including a heart that is fixing to get stopped. And it's going to kill that person.

    I have been 160/110 for 30 years.

My best friend is a 55 year old 30 year veteran Seattle EMT Fireman .

He just gave up Ironmans 15 years ago and triathlons 10 years ago but could still kick most "Athelectic" people's Azzazz these days has similar as mine.

I eat healthy , don't drink any soda or sweet crap. Just a gallon of water a day. I can out work most 25 year old kids in 97 degree heat.

Not overweight, energy of a Teen.

Did they already give you a 3 month card at first or did you just leave after you couldn't get you bo down. I'm going through the same exact issue. Mines was 155/86 and then when I checked at home it was 120/83. I no for sure it was anxiety and stress I had been checking it a week prior and it was not high at all

Only thing any of us needs is breathing exercises. You can bring your blood pressure down ALOT in 5 minutes.
I never have any problem with blood pressure anymore. Except when backing into a tight spot.
