Saw one like that the other day in Las Vegas on I-15.

Mixer trucks are always top heavy on the left side. As the drum turns, the product crawls up the left side until gravity overtakes the cement and it falls to the middle of the barrel. Many cement trucks have an extra wide front axle to the left to counter act the cement inside the mixer barrel.

Considering the non maintenance of CLEANING the Exterior, Lights as well Body, it would be no surprise of excess "Buildup" in Barrel to effectively turn over said vehicle, loaded or empty.
Strong Wind would only have helped.
Occasional application of Muriatic Acid, least once a week, helped to clean off exterior buildup with 1 company different than below.
ONCE, I was volunteered with another victim to get inside a Barrel and use a small air powered Jack Hammer to clean off buildup.
It was not repeated.
I drove such Trucks briefly yet turns were very s-l-o-w with 8-10 Yards at roughly 4K per yard.
Empty to yard I treated like Loaded.
