My shoulder seems to have the same problem, and it's been worse lately. I have an extended shifter but for the most part I can keep my elbow on the arm rest and just move my forearm to shift. Maybe it's time to try a shorty

I had a rotator cuff injury from moving to another truck and having the seat too far forward. I moved the seat back more. Therefore, when I move the shifter towards the rear of the truck it doesn’t hyper extend my shoulder. After a while it started getting better. I always now have the seat further back and the injury has not returned.

One sure sign of a torn rotator cup, is not being able to raise your arm (or elbow, with arm bent) any higher than your shoulder. I’ve had it once. Luckily it healed. Often they don’t and require surgery, before further damage results. Mine bothers me sometimes when shifting. It’s normal wear of the cup itself, from years of shifting. One thing that really aggravates it is sleeping with my arms above my head, either one on my side, or sometimes both lying on my back. Along with a habit of sitting with both hands behind my head. It’s a common injury for tennis players. The treatment? Keep arms below your head!!! Lol. Too simple. Anyways, I’ve known a couple of others who responded well to Cortisone shots, avoiding surgery. It can get bad enough, that lifting or even shifting is impossible. Mine comes and goes. Like many things. Heals up on its own. Raising my right arm now, my shoulders popping, as normal. Hopefully yours will go away, need to let it rest and heal.

Find a good chiropractor that specializes in soft tissue - suction cups help.
A good NSAID.
Look up adhesive capsulitis - I hope it’s not this - I’ve been there. It’s a long process, yours sounds like onset phase.
Thera band for stretching and strengthening. Amazon has it.
Me - flatbed and running with escorts - think grabbing mic 3,000 times in 10 hours, winch bar, shifting. I know exactly what you’re talking about.
