Air to Air, the incoming air flows through a heat exchanger (radiator) that the compressed hot air from the turbo charger is moving through. Air is removing the heat.
On an aftercooler, the compressed hot air also flows through a radiator, but it is cooled by the engine coolant, which at normal operating temperature is probably 180-190 degrees. That is as low as it can be cooled. On the air to air, it is able to cool it much lower. Cooler air, means more dense air for the pistons to compress, which means more fuel injected and more power.
On some of the earlier diesel turbos, there was no cooling the hot air from the turbos. It was just packing that super hot air into the engine. Like I stated, one reason today's diesels are putting down the power. Albeit, better and lighter oils, most are running light weight synthetics in the transmissions and differentials as well. Radial tires and aerodynamics have really helped, but I digress.
