OP, good thing you brought this up. Checked Washington State DOL and their website is wrong. Still says you have to carry medical card, which is no longer true. Will have to let them know.

Go to your local DMV office with your long form in hand , don't wast time calling and asking WTF. The will tell you what needs to be done.
In my state self certification is only for when you declare INTRASTATE or INTERSTATE
or making a change in that declaration.
Medical long forms must be brought in person as far as I know. Don't know if there's an option for faxing it to the main office for the state. Even if there was I would still as I've always walked it myself then you know for sure it's been scanned into the system.
Again contact YOUR local DMV. -

Favorite DMV story.
Sitting waiting for my number to be called and there is a guy at the counter who is there to get his license reinstated after a DUI. He is going through the motions looking a little shaky on his feet but it wasn't until his legs folded up and he fell, dragging the eye test machine off the counter with him as he went down that they came to the conclusion that he was probably over the limit. -
I normally try and set an appointment, but I have a 1 yr due to my BP Meds, so time passed by a bit fast and I missed the date. The DMV Office I use has a 3-4 hour lines outside just to get in the door to get a number to stand in line for the next available window.
I went in a few weeks ago to get my CDL A Permit, took 2 days. Day 1 was waiting so I could take the test, arrived at 8 am, stood inline until 11:30, finally was able to get a number and wait for it to be called. At 1:30 I was finally called to start the application process, at 3 I was able to start the written tests, had to take 9 total (Class C, Class A, Combo, Doubles\Triples, Passenger, Air Brakes, HazMat, Tanker and Motorcycle). I finished at 4:45, they said sorry, we stopped processing for the day, I needed to be back in line by 4. Day 2, I arrived at 7 am, there was already a line, but I was ale to get a number at a little after 10 am. Finally had permit in hand and out the door at 2 in the afternoon. It was 2 days spent in a place no one wants to be. If I were in a pinch I would call, the worse thing Sacramento can tell you, go spend an eternity in the line at DMV, or best thing, fax it in. While it is always better to stand in person (Still can't see the screen they are typing in, I have never received a receipt, they just take the long form and give me the full page should be wallet sized paper). I am suprised anyone can get anything done at DMV without an appointment, you could die of old age waiting in line.
