It is not the DOT. It is your state that will control that. They are mandated by the FMCSA to comply. If you don't self certify your state can downgrade your licence. Quit playing with a loaded gun. Just get the form and get it to your state.

I had that happen to me in pa at dod facility. After my dot physical, I personally took my new card to va dmv and they said they it would be updated. A year and a few months later, i was at dod facility and they said they were impounding my truck cause i wasnt a valid cdl driver. I had to call va dmv then find a fax machine to fax it over. Its way past time to drain the swamp....

Yup, plus I had the DMV guy go over it to make sure. He said everything is updated in the system and you have your class A back and your endorsements!

I know in NC they sent out letters and you had x days to comply. If you did not your license was downgraded to a "C" license.

Medical
All commercial drivers of vehicles in interstate commerce with a maximum gross vehicle weight rating of over 10,000 pounds (4,536 kilograms) are required to obtain and maintain a valid Medical Examiner's Certificate (ME Certificate) Commercial drivers who drive vehicles requiring a CDL have two additional requirements. On or before January 30, 2014, all CDL holders must declare to their State Driver Licensing Agency (SDLA) that they only operate or expect to operate commercially in 1 of 4 possible categories with their CDL. This process is called self-certification. For more information on the self-certification categories see the Self-Certification FAQ's.

Compliance
January 30, 2015 States must begin enforcing the regulation that paper copies of the Medical Examiner's Certificate are only valid for the first 15 days after their issuance

How do I determine which of the 4 categories of commercial motor vehicle (CMV) operation I should self-certify to with my State Driver Licensing Agency (SDLA)? -

If you apply for, or visit a DMV Office for an endorsement change, change in License Class, or apply for a CDL License or Permit, you sign a DL44C, which has the self certification statement included. The only time anyone would use the form you linked is if they change from an Non-Excepted Interstate to Non-Excepted IntraState, or vice versa and there is no change in License Class, or endorsements. FMCSA also has 4 classes for self certification, California only recognizes 2 and does not exempt any CDL holders from the DOT Medical Requirement.
Mine was done in 2009, well before FMCSA required it, and self-certifications that do not list the FMCSA regulatory references are accepted by the FMCSA as being compliant. They showed me the statement on an old State Medical Form they have digitized from March 2009, 8 years ago, and it was just a blip on the form, and only cited Ca State Law. So those rushing to sign theirs, you probably already have. The DMV can see the date of the old ones, and it is not reflected on the online version of your driving record. I was also told you can fax in your medical, since you have already self-certified there is no requirement to recertify. Basically Cal was compliant well before the Fed's required it, and you only have to certify once, and don't have to worry about it unless you change certification classes.Pedigreed Bulldog Thanks this. -
