I had my physical and my form/card filled out. The doctor did not fill out the back of the card. Who signs where it says "official signature" at the bottom? And do I do anything with the long form or just keep it with me in my truck?

Assuming the Doc signed properly, (he needs to sign the long form and wallet card) you carry the wallet card and keep your long form at home. The exception will be when you go to orientation that accepts a current physical. They will need to see it and make a copy. Also, most state DMV's need to have a copy on file to keep your CDL valid.

I don't know what the "official signature" is. The medical card has a place for the doctor to sign and also a place for the driver. You will need to have it attached to your CDL through your state DMV. You should keep the long form with your records at home. all companies will require a copy of the long form. I never give the original long form, but copy it and give them that. If they want an original, they can pay for the physical, as I see it. Most companies will pay for you to get a physical so they get the original, and you just get the card.
