As on July 1, 2017 IN raised their fuel 10¢ and no longer does the surcharge. You are credited just like most other states.

I've been at Mercer with a van for almost 2 years now and have hauled plenty of forklift loads and only a handful anywhere near $1.40 a mile. The cheap ones tend to be coast to coast, the more profitable ones more SE to midwest (and northeast, but who in their right mind wants those). In 2016 van loads were among the more profitable ones but in 2017 there was a lot of other freight that paid better for me.

I run forklifts when it makes sense to do it, usually to get home. Ive seen loads that are above 4500 with deadhead going to the west coast but its not enough to offset the load out of the west. Only good loads ive seen coming out of cali are exhibit loads and ive got better things to do than sit 12 hrs waiting to be loaded.
