From the Nevada CDL website...
"Road tests will be conducted in accordance with FMCSA-approved guidelines with the examiner monitoring the subject's performance from a second vehicle. This will remain in effect until September 30, 2020."
So how does this work? How do I know to turn right, stop, or whatever?

you are reading it wrong. There is more to it than that. You cant drive a cdl truck on the road with a learners permit without a licensed driver/examiner in the cab.

Road Test:
All applicants will be required to have a licensed driver that holds a valid CDL with same class and endorsements as the applicant, for the Road Test portion of the exam. Please be advised your appointment will be cancelled if you cannot provide a driver for the test, and rescheduling is subject to the DMV’s availability.
A Cell Phone with speaker capabilities is now required for the Skills Test. Applicants must have a charged cell phone capable of making and holding a phone call for the duration of the test (DMV is not responsible any cell phone charges). Incoming calls are not to be accepted during testing. - Applicant must verbally describe drive test maneuvers being performed.
