Only forgot to unhook airlines once.they came loose and slapped the back of the cab. Nowdays like so many othere I have a routine. 1st unhook airlines and pigtail and put them in holder,2nd pull pin and then crank down landing gear. Have High Hooked once and took a bit to get it out from under the trailer so now I have a routine for that also.Back up until I feel pressure If I don't feel pressure I get out and make sure trailer isn't to high.Once it locks in I give it a test tug.Get out hook up air lines and pig tail get under and double checkeck that king pin is locked in then raise landing geat and complete pre-trip on tral.Before I pull out give another test pull.

I've also done the "airlines thing" once. Middle of my routine, driver comes up to me asking about something (we were at a company yard at the time). After I assisted him, I calmly got back in the cab, started to pull out, and THWACK--THWACK!. Stretched out the lines a bit, but no damage.

As far as speaking the truth, next time you forget to update your logbook -pull over at a weigh station and ask them to give you a ticket because not telling the truth is lying))). -
i kept my log book updated.....back when i was younger, no i didnt...so yeah then i was lying....but for many years i have ran legal... and so no,i didnt "forget" to update it

I've also forgot to unhook the lines. They got damaged, I went and got replacements and fixed them. My bad, my fix, no harm no foul.

You could have just replaced those yourself in the truck stop parking lot and nobody would have known it happened.
You need to really pay attention to what you are doing, otherwise your career will be over! When you are backing, get out and look! There should be no excuse for a backing accident. You don't just back until you hit something! Get out and look!
When you are doing things like hooking/unhooking your trailer, have a system or order in which you do things. This way every single time you do it its done the same way and in the same order. When you are unhooking, get out of your truck and walk around the front of the truck toward the rear. Walk around the back of the truck then from there walk from the back of the trailer toward the front. The first thing you will come to is the dolly handle. Lower your landing gear... The next thing you will come to is the king pin release... pull the handle and release the kingpin.... after that the next thing you will come to is the glad hands and pig tail. Disconnect the glad hands and unplug the pig tail. Hang them up out of the way and then get in the truck. Once in the truck you dump the airbags then push in the tractor brake. Pull forward very slowly and drop the trailer. As soon as the trailer drops... STOP!!! Let the trailer settle for a second and then push the airbag dump back in. Let the airbags fill for a brief second and then pull away.
Do this in the same order every time and eventually you will do it without thinking about it. -
Been there done that too. My routine got fouled up, I had dropped and hooked into an empty but then someone told me I had a live unload and needed to rehook to the loaded trailer. I came back to unhook but everything was backwards as I was approaching the passenger side from the shipping office so the routine was different. I sent a breakdown message that the airline were 'leaking' lol because technically they were (not the whole truth nothing but the truth but close enough). I drove over to a nearby truck stop and bought new airlines (electric pigtail was OK) and put them on myself when this mobile fix-it guy shows up. He was almost in tears that he was going to lose out on a commission so I just had him give me the replacement airlines and charge the company for the work.
