My turn....2 stupid things in 20 something years........
1) Didn't set the brakes on an elevated scale so the load would settle down..... it rolled away while I was in the chicken house printing the ticket.
2) Got lost looking for I-95 and ended up on the front gate of Fort Stewart Ga. 5 days after 911 with a load of very bad sheet. -
Chalupa lol. I gotta similar story about an Air Force base, got lost. Ended up really REALLY near some B1 Bombers. They weren't happy, and they weren't playing either
Ever since then I am pretty scared to get any where near Military Basses of any sort.

A couple of months after I started driving I was on a 2 lane in NJ, at the end of it I was supposed to take a left. My now ex-hubby in jump seat. When I took the left it spilled me onto a 4 lane in business district with retainer wall between North and south. Didnt cut wide enough and trailer ended up stradling wall. Didnt damage anything but couldnt get out of it and those NJ people were getting really mad I was holding up traffic. Fire chief happened along. He stopped traffic on 1 side so I could back up, then drove to other side and stopped traffic so I coould pull truck out. Talk about feeling stupid, I Thanked him for his help.

the MOST agonizing thing I have done HAD to be not lowering the dang landing gear before dropping..... a 44K load of paper.....in the wrong hole

@Nitrogen You never know, lol. -
People smoke at the pumps regularly, something you need to get used to. I saw on mythbusters that fuel and gas both need a direct flame to ignite.

Here's another one I remembered from Lake Havasu. Pulled in and saw about 15 people looking in one direction, so I looked too. There in the parking lot was one of those double decker car transporters with a pick-up truck hanging, literally, by one wheel from the back end of the top level. The car in front of it was hanging by its back end off the other side of the same level.
I still don't know how he did it, unless he was coming around the corner too fast. -
careful at the pumps JB--with gasoline--it is the vapors that ignite--that is why it is worse on a hot day--with the vapors rising in the heat--all it takes is a bit of static electricity to make a fire
but with diesel it is the fuel itself that has to ignite--cant remember the exact temp for light off--but smoking beside diesel fuel isnt inherrently dangerous--can pretty much throw the butt in and it wont ignite--not so with gas
