Not so sure about Spirit. They say the reason the plug was removed was because of an issue with the seal...then someone neglected to put the bolts back in after they were done(maybe, or they were loose and fell out, then got sucked out when the plug blew out). United and Alaska airlines both have found more planes with loose bolts.

I can hear the tech now...sitting at the dinner table with the little woman, "I am thinking I forgot to do something, but I just can't remember what it was"

I saw an update that Spirit does their inspections one way where everyone signs off on whatever they touched, but Boeing does it a different way where they only sign off on new assembly installations, but stuff like the door plug being removed and reinstalled to fix something doesn't get a new inspection signature from everyone involved. Kinda spooky to think that they do it that way...hell, my Hanes underwear has an 'inspected by' tag

I’ve seen plenty of plane crash documentaries and quite a few of them were a result of faulty parts that flew under the rug (no pun intended). I know flying is such a common thing these days that people don’t realize that one 5 or 6 dollar bolt can bring an entire jet down. Little unnerving

The Concorde was taken down because some piece of metal, that probably was manufactured for chump change, fell from a thrust reverser of a DC-10 that took off just prior

Watched a YouTube video recently where a Fedex cargo plane had to make an emergency landing right after takeoff because one of their 2 engines suddenly decided to engage it's thrust reverser during takeoff.
