Not so sure about Spirit. They say the reason the plug was removed was because of an issue with the seal...then someone neglected to put the bolts back in after they were done(maybe, or they were loose and fell out, then got sucked out when the plug blew out). United and Alaska airlines both have found more planes with loose bolts.

I saw an update that Spirit does their inspections one way where everyone signs off on whatever they touched, but Boeing does it a different way where they only sign off on new assembly installations, but stuff like the door plug being removed and reinstalled to fix something doesn't get a new inspection signature from everyone involved. Kinda spooky to think that they do it that way...hell, my Hanes underwear has an 'inspected by' tag -
